For Physicians:
Dallas Dietitian Nutritionist is dedicated to developing a healthy partnership with physicians. Each patient has an ongoing relationship with a personal physician trained to provide first contact, continuous and comprehensive care. I understand the importance of the patient and physician relationship and I honor this relationship by keeping you informed about your patient’s progress.
The latest research shows that medical nutrition therapy provided by registered dietitians is linked to improved clinical outcomes and reduced costs related to physician time, medication use, and hospital admissions for people with obesity, diabetes, disorders of lipid metabolism, and other chronic diseases.
Medical nutrition therapy for diabetes and non-dialysis kidney disease is covered by Medicare when a Medicare beneficiary has been referred by a physician.
I want to make the referral process as easy as possible. The referral form can be downloaded here. The ICD-10 codes for Medical Nutrition Therapy are as follows:
- E11.9 Type 2 diabetes mellitus without complications
- E11.2 Type 2 diabetes mellitus with kidney complications
- N18.1 Chronic kidney disease, stage 1
- N18.2 Chronic kidney disease, stage 2
- N18.31 Chronic kidney disease, stage 3a
- N18.32 Chronic kidney disease, stage 3b
